access_db behaviour for a single user

Is there a way to have access_db type behaviour for only certain users
and not others?

I wish to block emails from certain domains and netblocks but I only
want to have my own account affected while leaving the others alone.
Normally I would be able to do this via procmail but that requires that
the mail be received. I'd rather have it happen at the time of
connection to save bandwidth and ensure that a proper rejection is
received by the sending MTA.

Is a milter what I need? If so, is it possible to write one that will
work only if the envelope contains a certain email address?

Would this be easier in a different MTA? I've only ever used Sendmail
but I'd be open to switching to something else if it made this sort of
thing easy.
